Follow the money far enough back and American medicine stops looking like a purely scientific enterprise and starts looking like an industry somebody built on purpose. Rockefeller built the largest private fortune of his era on refined petroleum. The Rockefeller Foundation itself was chartered in 1913.

He recommended consolidation around a research university model built on laboratory science, with Johns Hopkins as the template.

What followed reshaped the profession within a generation. Dozens of schools closed or merged. Licensing boards tightened. The surviving institutions were largely those able to attract the large endowments that Rockefeller and Carnegie money now supplied. Homeopathic colleges, eclectic colleges and other schools teaching botanical and manual traditions were among the heaviest casualties. Several medical colleges serving Black students also closed in this period, leaving Howard and Meharry as the two survivors, a consequence historians still weigh when they assess the reforms.

Treatments rooted in whole plants, diet, or practices outside the laboratory had no comparable path to revenue, and so they drifted from the center of professional attention toward the margins.

Proprietary schools sold degrees to students who had never touched a cadaver or a microscope. Standardization, laboratory training and grounding in germ theory produced measurable gains, and the antibiotic and vaccine eras that followed saved an enormous number of lives. On this reading Flexner was a reformer cleaning up a dangerous mess, and the money that funded the cleanup was money well spent.

Those two accounts are not as far apart as they first sound. Both agree the reforms concentrated authority. They disagree about what was lost in the process and about how much weight to give the commercial interests standing behind the reform. A viewer who holds both readings at once will get more out of this film than one who arrives already certain.

Most people encounter the pharmaceutical industry only at the pharmacy counter, as a price and a pill.
